با نهایت احترام به اطلاع همکاران رشتههای مختلف جراحی، متخصصین آسیبشناسی، رادیولوژی، انکولوژی، داخلی، اطفال، اعصاب و پزشکان عمومی، دکترای بهداشت و پزشک اجتماعی و پرستاری میرساند که به یاری خداوند متعال سی و هشتمین کنگره علمی سالانه جامعه جراحان ایران در 20 الی 24 اردیبهشت ماه 1393 در محل مرکز همایشهای زکریای رازی برگزار خواهد شد.
این کنگره علمی توسط انجمن جراحان ایران برگزار خواهد شد. هدف اصلی این کنگره و کنفرانس های علمی آن ایجاد فضایی علمی جهت تبادل اطلاعات بین جراحان سنتی و جراحان آندوسکوپیک لاپاروسکوپیست ایران میباشد.
در این کنگره علمی سعی بر این است که آخرین دستاوردها و تکنولوژیهای جراحی های سنتی (باز) و جراحی های آندوسکوپیک -لاپاروسکوپیک در جراحی ارائه شود و اطلاعات علمی و کاربردی مفیدی از یافتههای اخیر جراحی در اختیار شرکت کنندگان قرار گیرد. در این راستا، سخنرانیهای متعددی توسط جراحان متخصص و برجسته داخلی و خارجی در زمینه جراحی های مختلف من جمله لاپاروسکوپی ایراد میشود. همچنین دورههای فشرده تخصصی، جلسات بحث و گفتگو و ارائههای ویدیویی
نیز از برنامههای دیگر این کنفرانس علمی آموزشی میباشد .
دکتر نظری در این کنگره شرکت فعال دارند و در تعامل با دیگر جراحان از سراسر ایران، از پیشرفتهای روز جراحی نیز اطلاع حاصل مینمایند.

Abstract
BACKGROUND:
With the advances in flexible endoscopy, surgeons and gastroenterologists have been able to perform therapeutic procedures with instruments introduced through the working channel of flexible endoscopes. These procedures, however, have been mainly limited to technically minor ones, such as injection and cautery of bleeding ulcers and resection of polyps and small mucosal lesion (EMR & ESD). After the advent of Trans-Endoscopic Mucosectomy (TEM), intraluminal surgery developed. Progress in laparoscopic surgery and the employment of minimally invasive techniques have led to the emergence of a new branch in this field, laparoscopic endoluminal surgery (LES). Laparoscopic surgeons have been able to isolate the lumen of hollow organs as a separate working space. Laparoscopy-assisted intraluminal surgery is a novel alternative to open or laparoscopic surgery for a failed endoscopic endoluminal technique, minimizing the associated complications. Endoscopic resection of early cancers is restricted by the limited view of the endoscope, insufficient number of instrument channels, and inability to have adequate margins of resection without risking perforation. These cancers potentially can be treated by laparoscopy-assisted intraluminal surgery without resorting to major resection.
METHODS: LES encompasses all surgical procedures that involve intentional trocar, endoscope or instrument penetration into the lumen of the gut in order to perform a surgical procedure. The integrity of the organ is preserved except for the small enterotomy sites and the operated area. The procedure is technically demanding and requires sound advanced laparoscopic and endoscopic skills.
RESULTS: The experience in LES has primarily involved the stomach and right colon for two reasons. First, these organs have a larger volume/surface ratio in the gut; therefore, they are accessible for the endoluminal instruments. Second, even limited resection of the stomach or colon can potentially carry a significant morbidity. LES minimizes the extent of this resection. We report our results of LES regarding operations on stomach, small and large colon surgeries in 7 cases.
CONCLUSIONS:This procedure is relatively easy to perform and oncologically effective. In this paper we review recent developments in LES, discuss the technical aspects of the procedure, and recommend its applicability.
